can the four bolts that hold the plate be removed and mounted to another plate that is welded onto my current plate

Not sure if that'd be advisable or achievable, the plate is coupled with a coupler ring that enables the rotation of the jack. You may be better off securing the plate from the jack to the current plate on your trailer, or have the current one removed if needed.
